Contact form submissions are sent directly to Dr. Jarria by email so he can respond to you. Submissions are not stored in a database on this Site. Routine server logs (IP address, user agent, timestamp) may be retained for a short period for security and reliability purposes.
Please do not send PHI through this form
Do not include clinical details or protected health information (PHI) in inquiries submitted through this Site. Clinical communication should happen through the secure IntakeQ patient portal after you are an established patient.
Third-party services
The Site uses hosting (Vercel), email relay (Resend), and an embedded scheduling tool. Each service has its own privacy policy.
